T&M offers company paid Life and AD&D insurance at 2 times your earnings up to a maximum of $1,000,000. You can get up to $600,000 without completing an Evidence of Insurability form (EOI).

LONG-TERM DISABILITY - Provided at no cost to you

All active, full-time employees, regularly working at least 30 hours each week, are eligible for the Long-Term Disability

(LTD) Plan, available through Unum.

This is a Group Long-Term Disability Insurance plan provided to you by T&M, which provides a benefit of up to 60% of your base salary to a monthly maximum of $20,000.

 

Please Note: Life insurance elections may require Evidence of Insurability (EOI). EOI is an application process in which you provide medical information regarding the condition of your health to the insurance carrier for review.
