Voluntary Short-Term Disability
All eligible employees have the option to enroll in Unum’s Voluntary Short-Term Disability (STD) plan. After satisfying a 7 day elimination period, the STD benefit pays 66.6667% of weekly earnings to a maximum of $2,000 per week. Please Note: If you did not elect employee Voluntary STD insurance when it was first offered or if you change the benefit coverage amount, you will be required to submit an Evidence of Insurability (EOI) form. Voluntary Long-Term Disability
All active, full-time employees, regularly working at least 30 hours each week, are eligible for the Voluntary Long-Term Disability (LTD) plan, available through Unum. This plan is available in addition to the Group Long-Term Disability insurance already provided to you by T&M Associates, which provides a benefit of up to 60% of your base salary to a monthly maximum of $20,000. Please Note: If you did not elect employee and/or spousal Voluntary LTD insurance when it was first offered or if you change the benefit coverage amount, you will be required to submit an Evidence of Insurability (EOI) form.
